Q: how does this work with a cage?

The part shown here in their picture is on the inside of the cage. There is a metal part on the back that is bendable and you simply put it through the bars to hang. It can also work in glass aquariums. I hope that makes sense. I wish I could include a picture to show you.

Q: Can this hold a 4oz water bottle?

Yes. I have a little plastic one that's 4 oz.

Q: What is the diameter?

It will hold a standard small animal water bottle. I think those are an 8 ounce bottle.

Q: Will it hook over the side of a glass or plastic cage? The picture looks like it can't and just has a hole to bolt it to a cage that has bars.

Yes. It has a bendable metal piece on its back to slide over bars and rims. I'm guessing that hole is for hooking it to an S hook in case the back doesn't work out. Hope this helps.
